MedCadre is seeking a compassionate Licensed Practical Nurse (LPN) to deliver skilled home health care to geriatric patients in the Washington area. The role requires recent home health experience and proficiency with HCHB, ensuring accurate documentation and timely updates to the clinical team.

You will manage a caseload of about 57 patients per day, meet a 30-point weekly productivity target, and travel within a 60-mile radius.

Qualifikationen

• Active LPN license.
• ≥1 year of recent Home Health experience.
• Experience with Home Care Home Base (HCHB) is required.
• Current BLS certification.
• Ability to meet the weekly productivity requirement of 30 points.
• Reliable transportation and flexibility to travel within the assigned territory.
• Strong documentation, communication, and time-management skills.
• First-time travelers are not accepted.
