Job summary

AccentCare Inc. is seeking an LVN/LPN in Brenham to provide direct patient care under an RN's supervision. The role covers pediatric to geriatric patients with visits scheduled Monday–Friday, 8am–5pm, on-site only.

Ideal candidates are LVN/LPN graduates, licensed in the state, with at least one year of LVN/LPN experience, CPR certified, and willing to travel between locations. Benefits include medical, PTO, 401(k), and more, with compensation described as per-visit and an annual estimate

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Currently licensed as LVN/LPN in the state of operation.
• One year of LVN/LPN work experience.
• Provide nursing visits and maintain medical records.
• Valid CPR certification and ability to travel to all locations.

Responsibilities

• Performs nursing functions and direct patient care under RN supervision.
• Promote optimal client care under RN supervision.
• Maintain a safe patient environment.
• Identify abnormalities and communicate with case manager and team.
• Administer prescribed medications and assess drug therapy for issues.
• Re-evaluate nursing needs and report findings to Case Manager.
• Counsel and instruct client and family on nursing needs.
• Prepare clinical and progress notes; submit documentation timely.
