Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Licensed as LVN/LPN in the state of operation.
• At least one year LVN/LPN experience.
• CPR certification required.
• Valid driver's license and ability to travel to all locations.

Responsibilities

• Performs nursing functions and direct patient care under RN supervision.
• Promotes and maintains optimal client care under RN direction.
• Maintains a safe patient environment.
• Identifies abnormalities in patient data and reports to Case Manager.
• Administers prescribed medications and observes for adverse reactions and drug interactions.
• Re-evaluates nursing needs and reports findings to Case Manager.
• Counsels and instructs patient and family in nursing needs.
• Prepares clinical and progress notes.
• Submits documentation timely and aligns charting with the care plan.
• Communicates with home health team and coordinates services.
