Job summary

Sweetwater Care in Woodland, CA is seeking a licensed LVN to provide nursing care within scope of practice, following physician orders and center policies. You will participate in quality improvement, educate families and patients, and administer medications and treatments timely per facility policy.

Prior management experience is preferred but not required. Strong knowledge of long-term care and familiarity with PointClickCare charting will support coordination with the care team.

Qualifications

• Currently licensed as LVN in California.
• One-year prior nursing management experience preferred, not required.
• Long-term care experience.
• PointClickCare charting experience.

Responsibilities

• Provides nursing care within scope of practice according to standards and orders.
• Demonstrates knowledge and use of Infection Control Manual.
• Participates in Quality Improvement Program and identifies patient needs.
• Notifies departments of patient changes and admits/discharges.
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives and carries out physician orders as allowed by law.
• Works rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
