Job summary

HMG Healthcare is seeking a Licensed Vocational Nurse (LVN) for Park Manor Cypress Station in Houston, TX. The LVN will provide compassionate, skilled nursing care to residents in a long‑term facility, collaborating with RNs and physicians to implement care plans.

The role focuses on medication administration, monitoring patient progress, wound and catheter care, and meticulous documentation to ensure compliance and high quality outcomes. A TX LVN license and BLS are required.

Qualifications

  • Valid and current LVN license in TX.
  • Completion of an accredited vocational nursing program.
  • Experience in a long‑term skilled nursing facility or similar setting.
  • BLS certification.

Responsibilities

  • Administer medications and treatments as prescribed and per facility protocols.
  • Monitor and document patients’ vital signs and health status.
  • Develop and update individualized care plans with the team.
  • Provide wound care, catheter care, and other nursing procedures as required.
  • Communicate with patients, families, and interdisciplinary team for coordinated care.

About the Role:

The Licensed Vocational Nurse (LVN) plays a critical role in providing compassionate and skilled nursing care within a long-term skilled nursing facility. This position focuses on delivering high-quality patient care to residents with chronic illnesses, disabilities, or recovery needs, ensuring their comfort, safety, and well-being. The LVN collaborates closely with registered nurses, physicians, and other healthcare professionals to implement individualized care plans and monitor patient progress. Through meticulous documentation and adherence to healthcare regulations, the LVN helps maintain the facility’s standards of care and compliance. Ultimately, the LVN contributes to enhancing the quality of life for residents by addressing their physical, emotional, and medical needs in a respectful and professional manner.

Minimum Qualifications:

  • Valid and current Licensed Vocational Nurse (LVN) license in TX.
  • Completion of an accredited vocational nursing program.
  • Experience working in a long‑term skilled nursing facility or similar healthcare setting.
  • Basic Life Support (BLS) certification.
  • Strong understanding of nursing principles, patient care standards, and healthcare regulations.

Preferred Qualifications:

  • Experience with electronic health record (EHR) systems.
  • Additional certifications such as IV therapy or wound care specialty.
  • Previous experience working with geriatric or chronically ill patient populations.
  • Familiarity with state and federal regulations governing long‑term care facilities.
  • Excellent communication and interpersonal skills.
