Job summary

AMPLA HEALTH is seeking a dedicated LVN or RN to support clinicians and patients in a busy primary care clinic. You will take vital signs, collect specimens, perform tests, immunizations, and medications as ordered while aiding in education and care planning.

The role emphasizes accurate EHR documentation, adherence to PCMH protocols, and teamwork within the clinic. Candidates should have CPR and California licensure, with acute care and outpatient experience preferred.

Qualifications

  • Valid California LVN or RN license.
  • Current CPR certification.
  • One year of acute care experience preferred.
  • Experience with primary care outpatient procedures preferred.
  • Strong communication, organization, and time-management skills.
  • Ability to work effectively as part of a healthcare team.
  • Ability to maintain confidentiality of patient health information.
  • Knowledge of PCMH and clinic policies, procedures, and protocols preferred.

Responsibilities

  • Escort patients to exam rooms and obtain/document vital signs, height, and weight.
  • Collect and process laboratory specimens according to established protocols.
  • Perform diagnostic tests and administer immunizations and medications as ordered.
  • Assist providers with clinical messages, pharmacy communications, patient education, and care plans.
  • Initiate and update care plans and Advanced Directives in the EHR according to PCMH protocols.
  • Interpret PPD skin tests and document results.
  • Maintain accurate and timely EHR documentation and daily worklists.
  • Clean, sterilize, and maintain medical instruments and equipment.
  • Prepare and stock exam rooms.
  • Participate in staff meetings, in-service training, CQI, and quality care activities.
  • Maintain patient confidentiality and provide respectful, professional care.
  • Flexibility to work at other Ampla Health locations as needed.
  • Perform other duties as assigned.
