Job summary

Hurtt Family Health Clinic Inc is seeking a Licensed Vocational Nurse (LVN) to deliver high-quality ambulatory care in a federally funded health center setting.

Based in California, this full-time role supports clinical care delivery, patient flow, and back-office operations under the Nurse Manager and Clinic Manager. Bilingual Spanish/English required; schedule Monday–Friday 8am–5pm.

The LVN will participate in training, care coordination, and quality improvement initiatives.

Qualifications

  • California LVN license, current and in good standing.
  • Minimum 1 year of ambulatory care or community clinic experience.
  • Proficient in medical terminology and patient communication.
  • Bi-lingual in Spanish and English, both spoken and written.

Responsibilities

  • Provide patient care consistent with the California Nursing Practice Act and HFHC quality expectations.
  • Assist providers with examinations, screenings, procedures, immunizations, medication administration, and point-of-care tasks.
  • Collect and process laboratory specimens and support patient preparation.
  • Explain lab, imaging, and diagnostic results to patients per provider direction.
  • Document all care accurately in the EHR and ensure compliance with documentation standards.
  • Support onboarding, competency assessments, and training of medical assistants and LVN staff.
  • Manage provider message pools and triage non-clinical items to appropriate staff.
  • Coordinate laboratory and imaging orders, ensuring accuracy and clear patient instructions.
  • Monitor inventories, including supplies, medications, vaccines, and emergency kits; place orders.
  • Assist with vaccine program oversight, inventory tracking, and reporting.
  • Assist with medication refill requests and prior authorizations; facilitate communication with patients and pharmacies.
  • Facilitate referral communication and liaise with referral coordinators for timely follow-up.
  • Complete forms and care documentation; forward to providers within established timelines.
  • Contribute to quality improvement initiatives and participate in team huddles and staff meetings.
  • Perform additional duties as assigned by Nurse Manager or Clinic Manager to support goals and compliance.

Description

Under the administrative direction of the clinic manager, and the clinical direction of the Nurse Manager, the Licensed Vocational Nurse (LVN) provides high-quality ambulatory care within a Federally Qualified Health Center (FQHC) environment. This role supports clinical care delivery, enhances patient flow, and contributes to back-office operations, including clinical workflow leadership, training, and coordination of nursing support staff. The LVN ensures services are delivered safely, efficiently, and in alignment with organizational goals and regulatory requirements.


This position is full-time with a schedule of Monday through Friday 8am to 5pm.

Job Responsibilities & Duties


  1. Provide patient care consistent with the California Nursing Practice Act, Board of Nursing standards, and HFHC quality expectations.

  2. Assist providers with examinations, screenings, procedures, immunizations, medication administration, and point-of-care tasks within LVN scope of practice.

  3. Collect and process laboratory specimens, perform basic diagnostic tests, and support patient preparation.

  4. Explain lab, imaging, and diagnostic results to patients per provider direction.

  5. Document all care accurately in the electronic health record (EHR) and ensure compliance with clinical documentation standards.

  6. Act as a clinical resource for clinic managers and support staff, assisting with onboarding, competency assessments, and ongoing training of medical assistants and LVN support personnel.

  7. Manage provider message pools and triage non-clinical items to appropriate staff in a timely manner.

  8. Coordinate laboratory and imaging order placement, ensuring accuracy and clear patient instructions.

  9. Monitor clinical inventories, including supplies, medications, vaccines, and emergency kits; place orders and maintain par levels.

  10. Support vaccine program oversight, including ordering, tracking, and reporting inventory status to providers.

  11. Assist with medication refill requests, prior authorizations, and communication between patients, providers, and pharmacies.

  12. Facilitate referral communication and work with referral coordinators to support timely patient follow-up.

  13. Complete medical forms, letters, and care documentation, forwarding to providers within established timelines.

Job Skills & Requirements


  1. High School Diploma (or equivalent) and graduation from a California Accredited Licensed Vocational Nurse program.

  2. California State Board LVN License, current and in good standing.

  3. Minimum of 1-year experience in an ambulatory care or community clinic setting.

  4. Bi-lingual in Spanish and English, verbal and written communication with fluent knowledge in medical terminology.

  5. Current CPR for Healthcare Providers certification required.

  6. Ability to participate collaboratively with multi-disciplinary care team.

  7. Ability to work effectively with patients from diverse social, cultural and economic groups.

  8. Able to maintain good working relationships with employees, coworkers, and departments.

  9. Effective training and coaching skills; Knowledge of child, adolescent, and adult health care issues.

  10. Knowledge of, and ability to perform, clinic-based nursing tasks.

  11. Familiar with various funding sources such as Medi-Cal, Medi-Care, etc.

  12. Treat people with dignity, respect, compassion and integrity at all times.

  13. Ability to work in a fast-paced environment, and to multi-task.

  14. Excellent interpersonal, verbal and written communication skills.

  15. Demonstrated “detail-approach” to administration and organization.

  16. Basic computer literacy ability to comply with department needs and expectations (i.e., electronic medical record documentation, obtaining background information and reports on patients, following up on appts, etc).

  17. Basic typing skill (40 wpm) with proficiency with Windows Microsoft Office programs such as (Word, Excel, Outlook, Internet Explorer preferred but not required)

  18. Ability to recognize and maintain confidentiality of information as appropriate.

  19. Regular timely attendance.

While performing the duties of this Job, the employee is regularly required to walk and talk or hear.
