Health Link Home Health and Hospice is seeking a Full-Time Licensed Vocational Nurse (LVN) to provide compassionate, skilled care across San Francisco. You will collaborate with the Director of Nursing, clinical staff, and office teams to ensure coordinated, high-quality home health services.

The LVN role emphasizes wound care, IV certification, and patient education while supporting families in planning and executing care plans.

Qualifikationen

• Minimum 1 year of nursing experience in a medical setting.
• IV certification and experience with wound care.
• Home health experience recommended but not required.

Aufgaben

• Implement the plan of care initiated by the Registered Nurse
• Provide accurate and timely documentation consistent with the plan of care
• Assess and educate patients and caregivers regarding diagnosis, care plans, and treatments
• Participate in coordination of home health services, reporting additional needs (HHA, PT, OT, ST, MSW) to the RN or Clinical Supervisor
• Use equipment and supplies effectively and responsibly
• Complete online charting in Kinnser using company-provided devices
• Follow all company policies and procedures
• Perform other duties as assigned
