ANX Home Healthcare and Hospice Care is seeking a Licensed Vocational Nurse (LVN) for full-time roles in San Mateo County. Provide direct patient care in a home setting under RN supervision, delivering the care plan to support positive health outcomes.

Formación

• Current CA Licensed Vocational Nurse (LVN) license.

Responsabilidades

• Ensure high-quality patient outcomes by following the plan of care established by the RN Case Manager.
• Prepare complete and timely clinical notes and coordinate care with the interdisciplinary team.
