Job summary

Health Link Home Health and Hospice Agency in San Francisco, CA, seeks a full-time LVN to support home health care across the Bay Area. You will implement the physician's plan of care, document promptly, educate patients and caregivers, and coordinate with RN and field staff to ensure top-quality care.

Qualified candidates have at least 1 year of nursing experience, an active CA LVN license, CA driver’s license, and IV/wound care experience.

Qualifications

• Minimum 1 year of nursing experience in a medical setting.
• IV therapy certification/experience.
• Wound care experience preferred.
• Active California LVN license and valid CA driver's license.

Responsibilities

• Implement the plan of care initiated by the Registered Nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Assess and educate patients and caregivers regarding diagnosis, care plans, and treatments.
• Participate in coordination of home health services, reporting additional needs to the RN or Clinical Supervisor.
• Use equipment and supplies effectively and responsibly.
• Complete online charting in Kinnser using company-provided devices.
• Follow all company policies and procedures.
• Perform other duties as assigned.
