Job summary

Omega Home Health Care Services Inc in California is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care to homebound patients across Coachella Valley, including Palm Springs, Palm Desert, Cathedral City, Thermal, Banning, and nearby areas.

You will schedule visits, work with RN and physicians\' plans of care, document progress, and assist in patient education and self-care while maintaining high safety and professionalism.

Qualifications

• LVN license required.
• Driver's license required.
• Active BLS/ACLS/PALS.
• Reliable transportation.
• Ability to work independently with minimal supervision.
• Experience with electronic health records.

Responsibilities

• Provide skilled nursing care per the plan of care under RN/physician guidance.
• Promote patient health through teaching and rehab under RN supervision.
• Assist patients in learning self-care techniques.
• Coordinate care with other health professionals to ensure continuity.
• Prepare progress notes and follow the care plan.
• Assist physicians and RN with procedures as competency allows.
• Prepare equipment and observe aseptic technique.
• Keep up to date with changes in nursing care policies.
• Attend care conferences and communicate patient needs to the team.
