Omega Home Health Care Services Inc. is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care for homebound patients in Loma Linda and surrounding areas.

You will work under the guidance of an RN and physician's plan of care, delivering teaching and rehabilitative support to improve patient outcomes. Responsibilities include documenting progress, supporting care plans, and coordinating with health professionals.

Qualifications

• Active LVN license in California.
• Current BLS/ACLS/PALS certification.
• Experience in home health preferred.
• Ability to work independently with minimal supervision.

Responsibilities

• Provides skilled nursing care to clients as directed by the RN and physician's plan of care.
• Promotes client health through teaching and rehabilitative measures under the supervising RN.
• Assists clients in learning self-care techniques.
• Collaborates with other health professionals to maintain continuity of care.
• Prepares clinical progress notes and follows care plans.
• Assists physician and RN in performing procedures.
• Prepares equipment and materials for treatments observing aseptic technique.
• Keeps abreast of changes affecting nursing care and nurse functions.
• Attends client care conferences and communicates with the RN.
