Job summary

MyCHN Home Health Services is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care in patients' homes under RN supervision in the Greater Houston area.

The LVN will observe and assess patients, administer treatments within scope, educate patients and caregivers, and ensure compliance with Medicare/Medicaid and agency policies. Travel to patient homes across Brazoria, Galveston, and Harris Counties is required.

Qualifications

• Graduate of an accredited LVN program.
• Current Texas LVN license in good standing.
• Minimum of two years of healthcare experience; home health preferred.
• CPR or BLS certification required.
• Reliable transportation and ability to travel across service areas.

Responsibilities

• Provide skilled nursing care per physician orders and the Plan of Care.
• Work under the supervision of an RN.
• Observe, assess, and document patient status.
• Administer medications within LVN scope.
• Educate patients and caregivers on disease processes and home care procedures.
• Coordinate with interdisciplinary care team to ensure quality care.
• Maintain HIPAA compliance and agency policies.
• Document visits timely and accurately.
