Job summary

MyCHN Home Health Services is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in their homes under the supervision of an RN and physician orders. You will observe, assess, educate, and coordinate care to improve outcomes and independence for patients.

The role requires2+ years in healthcare (home health preferred), Texas LVN licensure, CPR/BLS, and reliable transportation for travel across Brazoria, Galveston, and Harris Counties.

Qualifications

  • Graduate of an accredited Licensed Vocational Nursing (LVN) program.
  • Current Texas LVN license in good standing.
  • Minimum of two (2) years of experience in a healthcare setting; home health preferred.
  • CPR or Basic Life Support (BLS) certification.
  • Reliable transportation and ability to travel throughout assigned service areas.

Responsibilities

  • Provide skilled nursing care to patients in their homes under physician orders and Plan of Care.
  • Work under supervision of an RN.
  • Collaborate with interdisciplinary care team to deliver patient-centered care.
  • Administer medications and treatments within LVN scope.

What You'll Do & Bring

As a Licensed Vocational Nurse (LVN), you will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN) and in accordance with physician orders and individualized Plans of Care. You will observe and assess patients, monitor progress, provide treatments within the scope of LVN practice, and educate patients and caregivers to support optimal health outcomes, independence, and quality of life. The LVN plays a vital role in coordinating care, promoting patient safety, ensuring compliance with agency policies and Medicare and Medicaid regulations, and collaborating with the interdisciplinary care team to deliver compassionate, patient-centered care.

Job Summary

Provide nursing care to patients in the home setting. Observe and assess the patient and/or caregiver to enhance quality of life and promote positive health outcomes. Demonstrate individualized creativity in educating patients and caregivers while following agency nursing policies and procedures. Provide care in accordance with physician orders and the established Plan of Care. Demonstrate an understanding of Medicare and Medicaid regulations and maintain compliance with applicable standards.

Duties and Responsibilities
  • Provide skilled nursing care to patients in accordance with physician orders, the Plan of Care, and agency policies.
  • Work under the direction and supervision of a Registered Nurse (RN).
  • Demonstrate effective teamwork and collaboration with clinical and administrative staff.
  • Perform nursing procedures according to agency policies and procedures.
  • Conduct patient observations, assessments, and evaluations to monitor patient progress and response to treatment.
  • Monitor for changes in patient condition and promptly report adverse findings to the RN and physician.
  • Administer medications and treatments as prescribed and within the scope of LVN practice.
  • Educate patients and family members regarding disease processes, medications, treatment options, home care procedures, safety practices, and the Plan of Care.
  • Reinforce teaching and care instructions established by the physician and RN.
  • Coordinate and monitor patient care and services with members of the interdisciplinary team.
  • Follow Medicare and Medicaid regulations and agency standards.
  • Maintain accurate, complete, and timely documentation of skilled visits in accordance with Medicare guidelines.
  • Maintain patient records according to agency policies and procedures.
  • Ensure compliance with HIPAA regulations both in and outside the office.
  • Follow infection control practices in all work settings.
  • Demonstrate strong organizational and time management skills.
  • Support quality improvement initiatives and agency performance goals.
  • Participate in case conferences, team meetings, in-services, workshops, seminars, and annual continuing education opportunities.

Required Education and Experience
  • Graduate of an accredited Licensed Vocational Nursing (LVN) program.
  • Current Texas Licensed Vocational Nurse (LVN) license in good standing.
  • Minimum of two (2) years of experience in a healthcare setting required.
  • Home health experience preferred.
  • Knowledge of home health regulations, standards of practice, Medicare, and Medicaid guidelines preferred.
  • Current CPR or Basic Life Support (BLS) certification required.
  • Reliable transportation and ability to travel throughout assigned service areas.

Angleton, TX 77515

This position requires regular travel to patient homes and assigned locations throughout Brazoria, Galveston, and Harris Counties.
