MyCHN Home Health Services in the Greater Houston area seeks a Licensed Vocational Nurse (LVN) to join our patient-centered home health team. You will provide skilled nursing care in patients’ homes under RN supervision, coordinate with the interdisciplinary team, and educate families to support safe, independent living.

This role offers flexible scheduling, travel across Brazoria, Galveston, and Harris Counties, and opportunities for growth in a mission-driven organization.

Qualifikationen

• Graduate of an accredited LVN program.
• Current Texas LVN license in good standing.
• Minimum of two years of experience in a healthcare setting; home health preferred.
• Current CPR or Basic Life Support (BLS) certification.
• Bilingual English/Spanish preferred.
• Reliable transportation and ability to travel throughout assigned service areas.

Aufgaben

• Provide skilled nursing care to patients in accordance with physician orders, Plan of Care, and agency policies.
• Work under the direction and supervision of a Registered Nurse (RN).
• Demonstrate effective teamwork and collaboration with clinical and administrative staff.
• Administer medications and treatments as prescribed within LVN scope.
• Educate patients and families regarding disease processes, medications, home care procedures, and Plan of Care.
• Maintain documentation of skilled visits in accordance with Medicare guidelines and agency policies.
• Ensure compliance with HIPAA regulations and infection control practices.
• Demonstrate strong organizational and time management skills.
• Participate in case conferences, team meetings, and continuing education opportunities.
