Job summary

GrandCare Health Services is seeking a part-time Licensed Vocational Nurse (LVN) to deliver in-home nursing care in the West LA area. You will work under RN supervision, provide patient education, and execute the plan of care for orthopedic and post-surgical patients at home.

Requirements include an active California LVN license, CPR certification, and 2+ years of direct patient care, preferably in home health. Competitive per-visit pay with drive time and mileage reimbursement is offered.

Qualifications

• Experience in home health nursing is required.
• Active California LVN license and CPR certification.
• Minimum 2 years direct patient care experience.

Responsibilities

• Deliver nursing care as directed by the RN and patient plan of care.
• Provide accurate, timely, and compliant documentation.
• Collect assessment data and communicate findings to RN and physicians.
• Collaborate with the interdisciplinary care team.
• Use clinical equipment safely and efficiently.
• Engage in ongoing professional development.
