Job summary

GrandCare Health Services is seeking a Licensed Vocational Nurse (LVN) for part-time home health coverage across South OC. The role emphasizes one-on-one patient care in patients' homes, with a per-visit pay model and opportunities to grow clinically.

Requirements include an active CA LVN license, CPR, 2+ years of direct patient care (home health preferred), and a valid driver's license with a reliable vehicle. Bathing and wound/ostomy care experience is required.

Qualifications

• Active California LVN license.
• Current CPR certification.
• 2+ years of direct patient care experience, home health preferred.
• Valid driver's license with reliable insured vehicle and good driving record.
• Willingness to perform bathing and wound/foley/ostomy care as needed.

Responsibilities

• Deliver nursing care under RN supervision per plan of care.
• Provide accurate, timely documentation.
• Collect assessment data and communicate findings to RN/physicians.
• Collaborate with interdisciplinary care team.
• Use clinical equipment safely and efficiently.
• Engage in ongoing professional development.
