Qualifications

• Graduation from an accredited School of Licensed Vocational Nursing.
• Two years of Subacute experience preferred.
• Long Term Care experience preferred.
• Current California licensure as LVN.
• Current Basic Life Support certification.
• IV therapy certification within six months of hire.

Responsibilities

• Care for all types of patients under the direction of an RN and within AHMC Seton Medical Center policies.
• Assess and document patient initial condition and ongoing changes; participate in care planning and evaluation.
• Plan and initiate patient teaching for illnesses; include psychosocial interventions with patients and families.
• Participate in discharge planning from admission through hospital stay.
• Administer and monitor medications as prescribed; document effects and any untoward events.
• Perform venipuncture and IV-related duties; follow hospital IV policies and document accordingly.
• Document information in the patient record accurately and timely; communicate with health care team.
• Demonstrate leadership by directing other health care team members; participate in orientation and training.
