Job summary

AHMC Seton Medical Center in Daly City seeks a Licensed Vocational Nurse (LVN) to provide appropriate nursing care under the direction of the Registered Nurse, in compliance with the LVN Nurse Practice Act and hospital standards. Demonstrate respect for patients and families.

Two years of SNF or subacute experience preferred. Requires CA LVN licensure, BLS, IV therapy coursework; participates in discharge planning and quality assurance; strong communication and teamwork are essential.

Qualifications

• Graduation from an accredited School of Licensed Vocational Nursing.
• California LVN licensure is required.
• Current Basic Life Support certification is required.

Responsibilities

• Provide nursing care under the direction of an RN in compliance with hospital standards and the LVN Act.
• Assess and document the patient’s initial condition and ongoing changes; participate in care planning.
• Administer and monitor medications as prescribed; document effects and respond to untoward events.
• Perform venipuncture and other nursing procedures as trained and approved by AHMC Seton Medical Center.
• Document patient records accurately and timely; participate in discharge planning and teaching.
• Engage in unit-based quality assurance activities and professional self-development.
• Communicate effectively with patients, families, and health care team; demonstrate leadership when directing other staff.
• Perform duties as assigned by an RN and contribute to safe, compassionate patient care.
