ANX Home Healthcare and Hospice Care in California seeks Licensed Vocational Nurses (LVNs) for full-time positions in San Mateo County, with opportunities also available in San Francisco County. You will provide direct patient care in home settings under RN supervision and document care plans to support positive patient outcomes.

Formación

• Current CA Licensed Vocational Nurse (LVN) license.
• Current IV & Blood Withdrawal Certification.
• Current CPR card for Healthcare Providers.
• 1 year of Licensed Vocational Nurse (LVN) experience.
• Must be a licensed driver with an automobile that is insured and in good working order.
• Demonstrates excellent verbal, written communication, and organization skills.

Responsabilidades

• Provide direct patient care and treatment in a home setting under the supervision of a Registered Nurse.
• Ensure high quality patient experience and positive health outcomes by delivering the plan of care established by the RN Case Manager.
• Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team (PT, OT, ST, MSW, and HHA).
• Licensed Vocational Nurse (LVN) positions are also available in San Francisco County.
