Job summary

Lone Star Circle of Care in Austin, TX is seeking a compassionate Licensed Vocational Nurse (LVN) to support patient care under supervision of licensed staff. You will perform focused nursing assessments, assist with examinations, manage medications, and document in the EHR.

Qualifications

• Graduate of an accredited vocational nursing program.
• Texas LVN license required.
• BLS certification required.

Responsibilities

• Obtain, evaluate, and document patient histories.
• Measure and record vital signs.
• Assist licensed medical staff with patient examinations.
• Draw blood samples and prepare specimens for lab analysis.
• Prepare and administer medications according to the Five Rights of Medication Administration.
• Perform routine diagnostic and laboratory procedures.
• Coordinate referrals and process referrals upon receipt of reports.
• Receive and relay STAT results to providers.
• Enter data in the EHR system including lab results and screenings.

A Day in the Life of an LVN

• Obtain, evaluate, and document patient histories
• Measure and record vital signs
• Assist licensed medical staff and/or Registered Nurses during patient examinations
• Draw and collect blood samples and prepare specimens for laboratory analysis as needed
• Prepare and administer medications according to the Five Rights of Medication Administration
• Perform routine diagnostic and laboratory procedures
• Manage referrals, including processing, tracking, and closing referrals upon receipt of consultation reports
• Receive and document STAT calls from Quest and ARA and communicate results to providers
• Complete data entry within the Electronic Health Record (EHR) system, including lab results, hemoglobin, and lead screenings
• Coordinate authorizations and identify in-network service providers for speech, occupational, and physical therapy referrals
• Perform other duties as assigned

Minimum Qualifications

• Graduate of an accredited vocational nursing program
• Current and valid Texas Licensed Vocational Nurse (LVN) license
• Current Basic Life Support (BLS) certification through the American Heart Association or American Red Cross

Preferred Qualifications

• Previous nursing experience in a clinic or outpatient setting
• Experience using Electronic Health Records (EHR)
• Bilingual English/Spanish skills
