Job summary

Monterey Park Hospital in the San Gabriel Valley seeks a Case Manager for the Utilization Review Department. This full-time, 8-hour day shift role reports to the Chief Operating Officer and involves Saturday through Wednesday scheduling.

The Case Manager will coordinate Continuum of Care for both adult and pediatric patients, initiate reviews, and ensure appropriate discharge planning with physicians, families, and care teams. Strong knowledge of Interqual/MCG guidelines is preferred.

Qualifications

• Current California RN license or CA LVN license required with 3-5 years case management experience.
• Experience in hospital case management and Utilization Review preferred.
• Familiarity with discharge planning processes and multidisciplinary coordination.

Responsibilities

• Obtain Continuum of Care functions for adults/pediatrics from admission to post-discharge placement.
• Initiate admission reviews, prioritize continue stay reviews, and review post-discharge plans with MD and family.
• Process TAR submissions and evaluate medical necessity using Interqual and MCG guidelines.
• Coordinate discharge planning and interface with health plans and social services.
