Job summary

Western Slope Healthcare Center in Placerville, CA is seeking a Licensed Vocational Nurse (LVN) for a full-time position with PM and NOC shifts. Starting pay is $36.00/hour DOE, with a supportive team and comprehensive benefits for eligible employees.

As an LVN, you will deliver direct nursing care, administer medications, supervise CNAs, and contribute to care planning and documentation. CA LVN license, valid CPR, and SNF/post-acute experience are preferred.

Qualifications

  • Current California LVN license in good standing.
  • Graduate of an accredited LVN program.
  • Current CPR Certification.
  • Skilled Nursing Facility or Post-Acute experience preferred.
  • Experience with PointClickCare (PCC) is a plus.
  • Excellent clinical judgment, communication, and organizational skills.

Responsibilities

  • Deliver direct nursing care in accordance with physician orders and individualized care plans.
  • Safely administer medications, treatments, and nursing procedures.
  • Supervise and support CNAs.
  • Monitor resident conditions and promptly communicate changes to physicians and the interdisciplinary team.
  • Complete accurate, timely, and compliant clinical documentation.
  • Admit, transfer, and discharge residents as needed.
  • Participate in care planning, quality assurance, and interdisciplinary meetings.
  • Maintain infection prevention and safety standards.
  • Support resident rehabilitation and restorative nursing programs.
  • Foster a positive, respectful environment for residents and families.
  • Provide leadership and mentorship to nursing assistants during shifts.

What You'll Do

As an LVN, you will play a vital role in providing exceptional nursing care while supporting residents, families, and fellow team members.

Your responsibilities include:

  • Deliver direct nursing care in accordance with physician orders and individualized care plans
  • Safely administer medications, treatments, and nursing procedures
  • Supervise and support Certified Nursing Assistants (CNAs)
  • Monitor resident conditions and promptly communicate changes to physicians and the interdisciplinary team
  • Complete accurate, timely, and compliant clinical documentation
  • Admit, transfer, and discharge residents as needed
  • Participate in care planning, quality assurance, and interdisciplinary meetings
  • Maintain infection prevention and safety standards
  • Support resident rehabilitation and restorative nursing programs
  • Foster a positive, respectful, and compassionate environment for residents and families
  • Provide leadership, mentorship, and guidance to nursing assistants throughout your shift
Qualifications

We're looking for candidates who have:

  • Current California LVN license in good standing
  • Graduate of an accredited LVN program
  • Current CPR Certification
  • Skilled Nursing Facility or Post-Acute experience preferred
  • Experience with PointClickCare (PCC) is a plus
  • Excellent clinical judgment, communication, and organizational skills
  • A compassionate, dependable, and team-oriented approach to resident care
