Job summary

Gallaher Signature Living seeks an experienced Wellness Coordinator to lead person-centered care in a luxury senior living community in California. You will ensure compliance with Title 22 regulations while coordinating ISPs, team communication, and resident wellness goals.

Reporting to the Wellness Director, you will collaborate with nursing, life enrichment, dining, and operations to deliver exceptional hospitality-driven care and maintain high quality of life for residents.

Qualifications

• Must hold a current LVN license.
• Minimum 2–3 years of experience in assisted living/RCFE or similar senior living.
• Knowledge of California Title 22 regulations for RCFE/community care.

Responsibilities

• Oversee resident care per Title 22 and regulatory requirements.
• Develop and review Individualized Service Plans (ISPs) based on assessments.
• Observe residents’ status and report changes to the Wellness Director and licensed professionals.
• Support residents in maintaining independence with safety, dignity, and choice.
• Coordinate care conferences and update care plans as needed.
• Provide guidance to care staff and assist with onboarding and training.
