Qualifications

  • Licensed Clinical Social Worker (LCSW) and/or Registered Nurse (RN).
  • At least 2 years of care management experience; 4-6+ years preferred.
  • Home health, discharge planning, or long-term care experience preferred.

Job description

Title: LTSS Service Care Manager

Location: Remote - Rocky Mount, NC residency preferred (within 2.5-3 hours radius from home residence for face-to-face member visits). Mileage and lodging (if needed) are reimbursed.

Duration: 6 Months (Potential to extend)

Shift: 8 AM - 5 PM

Job description:
  1. Managing a caseload for healthcare members with long-term care needs.
  2. Conducting monthly and quarterly member contact, including approximately 80% travel. Remote role requiring a valid driver’s license.
  3. Performing member assessments and documenting notes.
  4. Completing assessments with members, caregivers, or providers to gather information for care plan development.
  5. Monitoring service delivery and following up with members, caregivers, or providers through in-person visits and telephonic contact.
  6. Authorizing and coordinating referrals for services.
  7. Ensuring provider services are delivered without gaps and identifying functional deficiencies in care plans.
  8. Assisting in developing informal or voluntary services to integrate into the member’s care plan.
  9. Collaborating with discharge planners, physicians, and other parties to ensure appropriate discharge and care plans, and coordinating acute and long-term care services.
  10. Assisting members with filing and resolving complaints and appeals.
Qualifications:
  • Licensed Clinical Social Worker (LCSW) and/or Registered Nurse (RN).
  • At least 2 years of care management experience; 4-6+ years preferred.
  • Home health, discharge planning, or long-term care experience preferred.
  • Valid driver’s license.
Skills and Experience:
  • Experience with electronic medical records.
  • Proficiency in Microsoft Office.
Education:
  • Bachelor’s degree required.
  • Additional education preferred.

  • This is a remote position with travel required (within 2.5-3 hours of residence).
