Title: LTSS Service Care Manager
Location: Remote - Rocky Mount, NC residency preferred (within 2.5-3 hours radius from home residence for face-to-face member visits). Mileage and lodging (if needed) are reimbursed.
Duration: 6 Months (Potential to extend)
Shift: 8 AM - 5 PM
Job description:
- Managing a caseload for healthcare members with long-term care needs.
- Conducting monthly and quarterly member contact, including approximately 80% travel. Remote role requiring a valid driver’s license.
- Performing member assessments and documenting notes.
- Completing assessments with members, caregivers, or providers to gather information for care plan development.
- Monitoring service delivery and following up with members, caregivers, or providers through in-person visits and telephonic contact.
- Authorizing and coordinating referrals for services.
- Ensuring provider services are delivered without gaps and identifying functional deficiencies in care plans.
- Assisting in developing informal or voluntary services to integrate into the member’s care plan.
- Collaborating with discharge planners, physicians, and other parties to ensure appropriate discharge and care plans, and coordinating acute and long-term care services.
- Assisting members with filing and resolving complaints and appeals.
Qualifications:
- Licensed Clinical Social Worker (LCSW) and/or Registered Nurse (RN).
- At least 2 years of care management experience; 4-6+ years preferred.
- Home health, discharge planning, or long-term care experience preferred.
- Valid driver’s license.
Skills and Experience:
- Experience with electronic medical records.
- Proficiency in Microsoft Office.
Education:
- Bachelor’s degree required.
- Additional education preferred.
