Qualifications

• Bachelor's degree required.
• 1 year of related experience required.
• Licensure: Illinois RN, or LPN with elderly assessment experience, or Bachelor's/Master's in human services.

Responsibilities

• Evaluate member needs and facilitate the care plan.
• Develop ongoing long-term care plans/service plans.
• Coordinate between member/family and care team.
• Provide resource support to members and families.
• Monitor and adjust care plans per member needs.
• Interact with long-term care providers to meet needs.
• Collect and maintain member information.
• Perform on-site visits to assess needs.
• Educate members and families on care options.
• Provide feedback to leadership to improve care quality.

About This Role

The LTSS Service Care Coordinator assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. This role is ideal for candidates with case management, advocacy, or home visit experience.

Highlights

• Pay: $23 - $39/hour
• Location: Remote, US

• Schedule: Full-time, Day shift

What You'll Do

• Evaluate the needs of the member and recommend/facilitate the plan for the best outcome
• Assist with developing ongoing long-term care plans/service plans
• Coordinate between the member and/or family/caregivers and the care provider team
• Provide resource support to members and their families/caregivers
• Monitor care plans/service plans and provide recommendations based on member needs
• Interact with long-term care healthcare providers to ensure member needs are met
• Collect, document, and maintain long-term care member information
• Perform on-site visits to assess member's needs
• Provide education to long-term care members and their families/caregivers
• Provide feedback to leadership on opportunities to improve quality of care

Requirements

Education:

• Bachelor's degree required

Experience:

• 1 year of related experience required

Licenses & Certifications:

• One of the following licenses required: RN licensed in Illinois, OR
• Bachelor or Master’s Degree in a human services related field, OR
• LPN with one (1) year experience in conducting comprehensive assessments for the elderly
