Job summary

Molina Healthcare is hiring a Care Manager in Milwaukee, WI to support care management and LTSS activities. You will collaborate with a multidisciplinary team to coordinate member care across the continuum and help members progress toward outcomes.

Responsibilities include performing comprehensive assessments, developing care plans, monitoring progress, and facilitating waiver services. Travel up to 40% may be required based on state contracts.

Qualifications

• At least 2 years of health care experience including LTSS.
• Clinical licensure and/or certification required only if state mandate.
• Bachelor's degree in a health care field may be required depending on state.
• Valid driver's license, reliable transportation, and travel readiness.
• Demonstrated knowledge of community resources and care coordination.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including home visits.
• Develops and implements care plans with members, caregivers and health professionals.
• Monitors care plan effectiveness, documents interventions and goals.
• Promotes integration of LTSS and behavioral health services.
• Assesses medical necessity and authorizes waiver services.
• Facilitates ICT meetings for service approvals or denials.
• Identifies barriers to care and supports members to address obstacles.
• Travel up to 25–40% may be required depending on state contracts.
