Job summary

Molina Healthcare is seeking a Care Manager to support long-term services and supports (LTSS) and care coordination for high-need members. You will work with a multidisciplinary team to ensure integrated care across the continuum and progress toward desired outcomes.

The role involves developing care plans with members and caregivers, monitoring progress, and promoting service integration, including behavioral health and community resources. Travel up to 40% may be required.

Qualifications

• At least 2 years of health care experience, with LTSS exposure.
• 1 year in care management or medical/behavioral health setting.
• Licensure/certification only if required by state contract or licensing mandates.
• Drafts and implements care plans with stakeholders including physicians and families.
• Valid driver’s license and reliable transportation for travel reqs.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans addressing member needs and goals.
• Monitors care plan effectiveness and documents interventions and outcomes.
• Promotes integration of LTSS and behavioral health with community resources.
• Assesses medical necessity and authorizes waiver services.
• Facilitates ICT meetings for service approval/denial.
• Uses motivational interviewing and Molina guidelines to educate and motivate change.
• Identifies barriers to care and assists with psycho/social, financial, and medical obstacles.
• Identifies critical incidents and develops prevention plans.
• Collaborates with licensed care managers as needed.
• 25-40% local travel may be required.
