Job summary
Complete Care - New Jersey & PA is seeking an experienced Social Services Worker (SSW) with an LSW or CSW license for our long-term care facility in Wayne, NJ. This is a full-time role with potential weekend on-call.
The position requires 1–2+ years in an LTC/nursing home setting, an active NJ CSW/LSW license, and PCC experience. You will provide psychosocial support, develop care plans, coordinate with clinicians, and advocate for residents' rights and wellbeing.
Qualifications
- Must have 1–2+ years of prior experience as a Social Worker in an LTC/nursing home setting.
- Must have an Active/Valid NJ CSW/LSW License.
- Must have PCC experience.
- Strong communication and interpersonal skills.
- Compassionate, patient, and dedicated to improving quality of life for residents.
Responsibilities
- Provide psychosocial support to residents and their families.
- Develop and implement individualized care plans.
- Coordinate with healthcare professionals to ensure comprehensive care.
