Job summary

Concept Pharmacy is seeking a Full-Time Pharmacist (RPh or PharmD) to join our long-term care pharmacy team in Washington, PA. You’ll support skilled nursing, assisted living, and other healthcare facilities across Pennsylvania.

This is an excellent opportunity for a licensed Pennsylvania pharmacist interested in clinical medication management and collaborative work with healthcare providers and facility teams.

Qualifications

• Active, unrestricted Pennsylvania Pharmacist license (RPh or PharmD).
• Experience practicing as a licensed pharmacist in long-term care, institutional, or community settings.
• Knowledge of IV compounding and USP standards.
• Strong attention to detail and clinical judgment.
• Ability to work independently and effectively in a fast-paced pharmacy environment.

Responsibilities

• Review, verify, process, and dispense medication orders accurately and efficiently.
• Provide clinical pharmacy support to long-term care facilities and healthcare providers.
• Conduct medication regimen reviews and help identify medication-related concerns.
• Supervise IV admixture preparation within our sterile compounding environment.
• Support medication inventory management and delivery coordination.
• Serve as a pharmacy resource for facility staff and providers.
• Help ensure pharmacy operations meet applicable clinical, safety, and regulatory standards.
