Bluestone Physician Services is seeking a Care Coordinator in Minnesota to empower seniors to navigate their healthcare journey. You will manage DHS and CMS requirements for MSHO/MSC+ members while traveling to homes and facilities, with a hybrid work arrangement.

This full-time role emphasizes patient-centered care, collaboration with multidisciplinary teams, and strong documentation. Local travel and home visits are essential, with regular weekday hours.

Qualifications

• Current MN Licensed Social Worker (LSW/LMSW/LISW or LICSW) credential is required or new grads encouraged to apply.
• Minnesota Registered Nurse license is required or preferred.

Responsibilities

• Coordinate face-to-face visits with members.
• Manage Elderly Waiver and care plans.
• Conduct annual PCA assessments and living tools for members in assisted living.
• Support transitions of care and discharge planning.
• Document all activities and maintain compliance with DHS and CMS requirements.
• Collaborate with care teams to meet quality measures.
