The role requires a valid LSW license and at least 2 years of relevant experience, with strong communication, organizational, and EMR skills.

Qualifications

• LSW license required.
• Minimum 2 years in admissions, social services, case management, discharge planning, or related health care field preferred.
• Solid knowledge of Medicare, Medicaid, managed care, and insurances.
• Experience with skilled nursing facility admissions, reimbursement systems, and long-term care regulations.

Responsibilities

• Triage referrals and coordinate clinical reviews to support admission decisions.
• Verify payer source and obtain authorizations.
• Collaborate with nursing leadership to assess clinical appropriateness and facility capacity.
• Serve as liaison with hospitals, case managers, discharge planners, and referral sources.
• Communicate bed availability, resident status, and potential long-term care conversions to Social Services.
• Support discharge planning and care transitions for skilled residents.
• Maintain referral, admission, census, and occupancy records and use EMR systems.
• Develop and implement outreach strategies to increase skilled admissions and strengthen referral partnerships.
