Job summary

Independence Home Health LLC is seeking nurses to deliver high-quality home care Friday-only shifts in Franklin, Indiana. The roles require graduation from an accredited nursing program and preferred home health experience.

Candidates must demonstrate effective communication, patient assessment, and collaboration with RNs, LPNs, and home health aides within agency policies and safety standards.

Qualifications

• Graduate of an accredited school of professional nursing.
• Home Care Experience preferred.
• Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice.
• Communicates effectively and in accordance with Agency requirements.
• Assess’ the continuing needs of the patient. Performs initial and ongoing patient’s assessments based on Agency policy and standards of practice to ensure effective and appropriate home care services, as needed.
• Participates with team of RNs, LPNs and or Home Health Aides to provide effective and quality home care services.
• Promotes personal safety and a safe environment for patients and coworkers.
• Performs job duties in accordance with Agency policies, procedures, and professional and community standards.

Responsibilities

• Assess’ the continuing needs of the patient and perform initial and ongoing assessments as per Agency policy.
• Work with a team of RNs, LPNs, and Home Health Aides to deliver quality home care services.
• Promote safety for patients and coworkers and adhere to policies and standards.
