Job summary

Connecticut Institute For Communities, Inc. (CIFC) through CIFC Health's Women’s Health Department is seeking a licensed practical nurse (LPN) to join our team on a part-time or full-time basis.

You will work alongside doctors, nurse practitioners and other healthcare professionals to provide the highest standard of care to our patients. Qualifications

• High School Diploma or GED is required.
• Current LPN license in the state of Connecticut.
• Experience in pediatrics preferred.
• Strong communication and interpersonal skills; bilingual encouraged.
• Ability to work in a fast-paced environment.
• Attention to detail and multitask.
• Understanding of medical terminology and nursing protocols.
• Experience with electronic medical records (eClinicalWorks) preferred.

Responsibilities

• LPNs gather information for providers in assessment and patient care.
• Administer medications and treatments as prescribed by providers.
• Monitor and record vital signs, medical history and symptoms.
• Assist with physical exams and medical procedures within licensure.
• Provide education to patients and families on healthcare topics.
• Maintain accurate and timely documentation of patient care.
• Collaborate with school nurses, WIC colleagues, DCF and other medical departments; complete required paperwork.
• Triages for calls using protocols to determine appropriate care and timing of visits.
• Provide education on after-hours triage and referral services, and disease states.
• Contact specialists to obtain letters/results and help set up home therapies when appropriate.
