BrightStar Care is seeking a Licensed Practical Nurse (LPN) for in-home, one-to-one care in Illinois. The role offers hourly pay and a flexible schedule for an immediate placement on a patient-care team providing 24/7 support.

The successful candidate will hold an LPN license, CPR, and TB test results, with at least one year of nursing experience in home health or related settings. A valid driver’s license and ability to lift up to 50 pounds are required.

• Active unrestricted LPN license in the care-providing state.

• Ability to lift up to 50 pounds and assist with mobility.

Pay:$40.00 - $45.00 per hour
Job description:
We are looking for four candidates to fill an immediate position for one of our patients, in their home. This client receives 24/7 care and we are seeking qualified and caring nurses to join the care team.
Responsibilities

• Provide thoughtful care to the patient based on personalized Plan of Care
• Monitor and track changes in the patient’s conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
• Proactively updates patients’ family on plan of care, health changes, and concerns

Requirements

• Current unrestricted RN or LPN license in the state for which care is being provided
• Certified CPR and negative TB skin test or chest x-ray
• Minimum one (1) year of documented experience required in a home health agency, primary care clinic or health facility preferred.
• Licensed driver with valid automobile insurance
• Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds

Skills & Qualifications

Licensed Practical Nurse (LPN) Qualifications:

• An unencumbered license as a Licensed Practical Nurse (LPN) in the state where care is provided
• Minimum of (1) year nursing experience
• Excellent problem-solving skills and organizational and multi-tasking skills
• In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination, etc.) and willingness to follow them at all times
• Valid Driver's License, Auto Insurance, BLS CPR card, and an up-to- date TB skin test or Chest X-Ray
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull
• Must be able to lift or transfer up to 50 pounds
