As an LPN, you will deliver skilled nursing under the direction of a Registered Nurse, ensuring safe, compliant, patient-centered services aligned with state scope of practice and BrightStar Care standards.

Qualifications

• Active LPN license in good standing.
• Minimum of 1 year of clinical nursing experience; home health preferred.
• Strong clinical assessment and documentation skills.

Responsibilities

• Provide skilled nursing care per Nurse Practice Act and BrightStar Care policies.
• Implement the plan of care established by the Registered Nurse.
• Perform all LPN-permitted skills, treatments, and procedures safely.
• Deliver services requiring substantial and specialized nursing skill.
• Recognize changes in patient status and implement emergency measures promptly.
• Document care accurately and timely following the nursing process.
• Communicate changes in condition with the Director of Nursing and team.
• Support medication administration and wound care within LPN scope.
• Educate patients and families on care instructions and safety.
• Promote patient comfort, autonomy, and high-quality outcomes at home.

As an LPN on our team, you will deliver essential skilled nursing care under the direction of a Registered Nurse, ensuring safe, compliant, patient-centered services aligned with state scope of practice and BrightStar Care standards.

Key Responsibilities – Licensed Practical Nurse (LPN)

• Provide skilled nursing care in accordance with the Nurse Practice Act and BrightStar Care policies, procedures, and clinical protocols
• Implement the plan of care and treatment established by the Registered Nurse
• Perform all LPN-permitted skills, treatments, and procedures safely and competently
• Deliver services requiring substantial and specialized nursing skill
• Recognize changes in patient status and implement emergency measures promptly and appropriately
• Document care accurately, thoroughly, and in a timely manner following the nursing process and plan of care
• Communicate effectively with the Director of Nursing, Nurse Managers, physicians, and interdisciplinary team regarding changes in condition, progress, or patient needs
• Support medication administration, wound care, injections, disease monitoring, specimen collection, and other skilled tasks within the LPN scope
• Educate patients and families on care instructions, safety, and health management
• Promote patient comfort, autonomy, and high-quality outcomes in the home

Qualifications – LPN

• Active LPN license in good standing
• Minimum of 1 year of clinical nursing experience (home health preferred but not required)
• Strong clinical assessment and documentation skills
• Ability to work independently in a variety of home environments
• Excellent communication, professionalism, and reliability
• Competent in performing skilled nursing procedures within the LPN scope of practice
