Responsibilities

Job Summary

The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes. Responsibilities include recording clinical notes and coordinating patient care activities to ensure high-quality care within the geographic service area. She/he works under the supervision of the Registered Nurse or designated supervisor, adhering to state laws, licensure requirements, and agency policies.

Specific Job Duties/Responsibilities

• Provides high-quality clinical services within the scope of practice, following the plan of care and infection control procedures.
• Documents patient visits thoroughly in the electronic medical record and electronic visit verification system.
• Participates in care coordination, reporting patient needs and changes, attending case conferences, and communicating effectively with patients, families, physicians, and team members.
• Maintains confidentiality of patient information per agency policy.

Education and Experience

License Requirements: Current LPN licensure in Louisiana, with no restrictions. Must have a valid driver’s license, vehicle insurance, access to a dependable vehicle or public transportation, and current CPR certification.

Experience Requirements: Previous home care experience preferred.

Additional State-Specific Requirements: At minimum, one year of LPN experience in Louisiana.
