Morristown Post Acute Rehabilitation & Nursing Center is seeking a dedicated Licensed Practical Nurse (LPN) to join our team for weekend shifts. You will provide direct patient care, perform medication administration, monitor vital signs, and contribute to individualized care plans in collaboration with a compassionate interdisciplinary team.

Qualifications

• Licensed Practical Nurse (LPN) with current NJ license.
• Provide direct patient care and monitor vital signs.
• Assess patients and develop care plans with the team.
• Maintain accurate patient records using EHRs.

Responsibilities

• Provide direct patient care, including medication administration and wound care.
• Conduct initial and ongoing assessments of patients' physical and emotional needs.
• Develop and implement individualized care plans with the interdisciplinary team.
• Administer medications and treatments as ordered by physicians.
• Maintain accurate and detailed records of patient care and progress.
• Communicate effectively with patients, families, and healthcare team members.
• Participate in quality improvement initiatives and maintain a safe, clean environment.

About the Role:

We are seeking a dedicated and caring Licensed Practical Nurse (LPN) to join our dynamic team. As an LPN at Morristown Post Acute, you will be responsible for providing high-quality patient care, conducting assessments, and developing individualized care plans. You will work closely with our interdisciplinary team to ensure that each patient receives the best possible care and achieves their optimal level of health and function.
