Job summary

Home Health Care, Inc. is seeking a Full-Time RN Visit Nurse to provide skilled nursing visits across Mankato and surrounding Southern Minnesota communities.

You’ll assess, educate, and manage patient plans in patients’ homes, coordinating with a multidisciplinary team to reduce hospital readmissions and support transitions home.

Job Title: RN Visit Nurse | Mankato & Southern MN Home Health | $4,000 Sign-On Bonus

Location: Mankato, MN & Southern Minnesota Communities

Schedule: Full-Time with On-Call and Weekend Rotation

Compensation: $75,000 to $100,000 per year plus a $4,000 Sign-On Bonus

Home Health Care, Inc. is looking for a Full-Time RN Visit Nurse to provide skilled nursing care throughout Mankato and surrounding Southern Minnesota communities .

What You'll Be Doing

• Provide skilled RN home health visits throughout your assigned territory
  
• Perform wound assessment and wound care
  
• Provide medication reconciliation and management
  
• Administer injections and other skilled nursing treatments
  
• Monitor acute and chronic medical conditions
  
• Identify and respond to changes in patient condition
  
• Educate patients and caregivers about medications, diagnoses, treatments, and recovery
  
• Coordinate care with physicians and healthcare providers
  
• Collaborate with PT, OT, SLP, social work, and interdisciplinary team members
  
• Support patients transitioning home following hospitalization or rehabilitation
  
• Help reduce avoidable emergency department visits and hospital readmissions
  
• Complete timely documentation using Homecare Homebase (HCHB)

Who We're Looking For

We're looking for a nurse who is comfortable being both independent and accountable .

You’ll need:

• An active Minnesota Registered Nurse (RN) license
  
• Previous experience in home health, hospital, acute care, rehabilitation, skilled nursing, or another clinical environment
  
• Strong assessment and critical-thinking skills
  
• Confidence making clinical decisions in the field
  
• Excellent patient and caregiver communication
  
• Strong organization and time-management abilities
  
• Ability to manage a daily schedule of patient visits
  
• Reliable transportation and willingness to travel throughout the Southern Minnesota service area
  

Previous home health experience is certainly valuable, but it isn't the only background that can prepare you for this position.
