Job summary

Apex Home Health, Hospice & Home Care is seeking Licensed Practical Nurses to provide in-home visits, including medication administration, insulin assistance, wound care, and patient education. The role emphasizes evening and weekend hours to support patients in Newark, OH and surrounding areas.

Job description

Qualifications include a current LPN license, health certificate, PPD or chest X-ray, and BLS certification.

Key duties encompass patient assessment, medication management, care documentation, and patient education.

There are different team tiers: Tier 1 requiring availability for 4 days/week with at least 32 visits, and Tier 2 requiring 3 days/week with a minimum of 24 visits. A casual/PRN option is also available without set availability.
