Transitions Hospice is seeking a flexible LPN to join our Extended Care Team on a PRN basis in Champaign, IL. You’ll provide skilled nursing care across cases, assess patient needs, administer medications, and coordinate with physicians to ensure timely interventions while traveling to local patient homes.

Ideal candidates hold an active LPN license, hospice/home health experience preferred, strong documentation, and a willingness to travel locally.

• Graduate of an accredited school of Nursing.
• Clinical hospice/home health experience preferred.
• Flexibility across varied patient assignments.
• Strong assessment skills and clinical judgment.
• Excellent documentation and communication skills.
• Valid driver’s license and willingness to travel locally.

Responsibilities

• Deliver nursing care across the full nursing process within scope.
• Assess patients and contribute to care plans.
• Complete all visit types as needed to meet patient needs.
• Administer medications and treatments per orders.
• Identify nursing diagnoses from assessments and respond.
• Record timely clinical notes including pain/symptom outcomes.
• Communicate changes to physicians and obtain orders.
• Coordinate with community partners for seamless transitions.
• Ensure equipment and services are arranged for patients.
• Maintain and grow clinical skills through continued education.

What You’ll Do

As a Transitions Hospice ECT Nurse, you’ll work as a vital support member of the interdisciplinary care team — stepping in across cases to assess, treat, and communicate patient needs with speed and clinical precision. Your responsibilities include:

• Delivering professional nursing care across the full nursing process within your scope of practice
• Conducting thorough patient and family assessments and contributing to individualized care plans
• Completing all visit types as needed to meet patient and family needs
• Administering medications and treatments per physician orders
• Identifying nursing diagnoses from health assessment data and responding accordingly
• Recording accurate, timely clinical notes including pain and symptom management outcomes
• Communicating changes in patient condition to physicians and obtaining orders as required
• Coordinating with community health partners to support seamless care transitions
• Ensuring equipment and necessary services are arranged and available for patients
• Maintaining and growing your clinical skills through continuing education and in-service participation
