Job summary

Complete Care seeks a dedicated LPN Treatment Nurse to assess and implement wound care plans, delivering direct nursing care under physician and nursing leadership. You will prioritize tasks, educate patients and families, and ensure HIPAA-compliant documentation in a supportive long-term care environment.

The role emphasizes resident-centered care with opportunities to advance as part of a compassionate caregiving team. Illinois RN/LPN credentials and wound care experience are required.

Qualifications

• Associates or Bachelor’s Degree in Nursing.
• Current, Unencumbered Illinois LPN License.
• A minimum of one year of wound care and treatments experience.
• Prior experience/certification in Wound Care preferred.
• Long-term care experience preferred but not required.
• Proficient with basic computer skills.
• Clear written and verbal communication for documentation and education.
• Familiarity with completing the MDS assessment tool.
• LPN status may be considered.

Responsibilities

• Assess the unique needs of each patient to determine treatment plans.
• Provide direct nursing care under supervision and medical direction.
• Prioritize daily tasks to ensure timely and effective resident care.
• Modify treatment regimens to meet physical and psychological needs per policies and standards.
• Educate patients and families on healing and wellness practices.
• Track treatments and progress per HIPAA guidelines.
• Oversee non-licensed staff performance.
• Perform any other duties assigned by the Director of Nursing.

Qualifications

• Associates or Bachelor’s Degree in Nursing

• Current, Unencumbered Illinois Registered Nurse (LPN) License

• A minimum of one (1) year of experience in wound care and treatments

• Prior experience/certification in Wound Care is preferred

• Previous long-term care experience is preferred but not required

• Computer Knowledge

• Clear written and verbal communication skills (Essential for documentation and patient/family education)

• Familiarity with completing the Minimum Data Set (MDS) assessment tool.

• Licensed Practical Nurse (LPN) may be considered

Responsibilities

• Assess the unique needs of each patient to determine appropriate treatment plans.

• Provide direct nursing care to residents under the medical direction and supervision of the residents' attending physicians, the Director of Nursing Services, and the Medical Director of the facility.

• Ability to prioritize daily tasks to ensure timely and effective resident care.

• Assist in modifying the treatment regimen to meet the physical and psychological needs of the resident, in accordance with established medical practices and the requirements of the policies and procedures of this facility, and in accordance with current federal, state, and local standards governing the facility.

• Educate patients and families on best practices to promote healing and wellness.

• Track treatments and patient progress according to HIPAA guidelines.

• Monitor the performance of non-licensed personnel.

• Any other duties assigned by the Director of Nursing.
