Job summary

Sweetwater Care in Atlanta, GA is seeking an LVN/LPN to provide nursing care within scope of practice, following physician orders and center policies. This role ensures patient needs are met in alignment with standards of care and regulatory guidelines.

The ideal candidate holds a valid LVN/LPN license in Georgia, has Long Term Care experience, and is certified in BLS/CPR. Experience in nursing management is a plus but not required, with opportunities for professional growth.

Qualifications

• Experience in Long Term Care.
• Proficient with PointClickCare charting.
• Licensed LVN/LPN in practicing state.
• Current BLS/CPR certification or willingness to obtain.
• Nursing management experience is a plus.

Responsibilities

• Follow charting and incident reporting procedures.
• Follow procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Document changes in patient conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilize the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Make rounds with physicians when necessary.
