Job summary

Sweetwater Care is seeking a dedicated licensed vocational nurse (LVN) to provide patient-centered nursing care within scope of practice, following physician orders and center policies. The role requires LTC experience, PCC charting familiarity, and compliance with state and federal guidelines.

The incumbent will document patient changes and assist in rounds with physicians as needed. Qualifications include an active LVN/LPN license in the practicing state and BLS/CPR certification, with nursing

Qualifications

• Long-term care experience required.
• Current BLS/CPR certification or willingness to acquire one.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
