Job summary

Sweetwater Care is seeking an LVN/LPN to provide nursing care within scope of practice per standards and physician orders. You will follow center policies and applicable guidelines to meet patients’ needs in a skilled long-term care setting.

The role requires current LVN/LPN license, BLS/CPR, and prior long-term care experience; PCC charting experience is a plus, and leadership experience is preferred but not required. Equal opportunity employer.

Qualifications

• Point Click Care charting experience preferred.
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire.
• Experience in nursing management is preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutrition, infections, skin conditions or weight loss.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
