Job summary

Creekside Center for Rehabilitation and Healing is hiring an LPN to provide post-acute care across physical, occupational and speech therapies. The role is available day and night, full-time or part-time, with per diem options.

Responsibilities include medication administration, charting, and resident education. Applicants must possess a nursing degree from an accredited program or be graduate of an approved LPN/RN program.

Qualifications

• Must have nursing degree from an accredited college or university or be graduate of an approved LPN/RN program.

Responsibilities

• Medication administration and treatments.
• Assist with admission, transfer and discharge of residents.
• Document resident care in medical records and department records.
• Educate residents and families about treatment and status changes.
• Adhere to infection control policies and facility procedures.
• Consult with physician regarding plan of care and changes.
• Maintain resident confidentiality and privacy.
• Ensure quality of care per DOH and facility standards.
• Collaborate with interdisciplinary team to solve resident problems.
