Beth Israel Lahey Health in Vermont is seeking a licensed Practical Nurse to join our care team. The LPN will perform professional nursing care under the supervision of an RN Case Manager, participate in assessments, planning, and evaluation for assigned patients, and support educational needs of patients and families. Formación

• Active Massachusetts LPN license.
• Graduate from an NLNAC-accredited LPN program.
• Active BLS certification.
• 2+ years of LPN experience in acute care, rehab or home care.

Responsabilidades

• Perform nursing visits to patients as assigned by the RN Case Manager.
• Provide nursing care using current principles and symptom management.
• Observe and report significant changes to MD and RN Case Manager.
• Attend regular IDT meetings and maintain HIPAA compliance.

Conocimientos

LPN license

BLS certification

Acute care experience

NLNAC-accredited LPN program
