Qualifications

• Active American Heart Association BLS certification.
• 2+ years of experience as an LPN in acute care, rehab or home care.

Responsibilities

• Provide professional nursing care as part of the team under RN supervision.
• Perform nursing visits to assigned patients and contribute to assessment, planning and evaluation of care.
• Observe, document, and report significant changes in patient health to MD and RN Case Manager.
• Contribute to health teaching for patients and families and respect cultural beliefs and preferences.
