Job summary

Signature HealthCARE, LLC is seeking an experienced LPN/LVN to join our McCreary County Rehab & Wellness Center team. Provide direct nursing care, administer medications, and support residents in a 60-bed facility focused on rehabilitation and long-term care.

The successful candidate will supervise nursing assistants, maintain accurate documentation, and collaborate with an interdisciplinary team to develop individualized care plans that promote comfort, safety, and quality of life.

Qualifications

Active LPN/LVN license in the practice state.
CPR/BLS certification (preferred).
Minimum 1 year clinical nursing experience; long-term care experience preferred.
Supervisory or charge nurse experience preferred.
Experience with EHR and documentation.

Responsibilities

Provide direct nursing care to residents, including medications, wound care, and skilled nursing procedures.
Supervise nursing assistants and medication aides during shifts.
Monitor and observe residents; notify physician of changes in condition.
Develop, implement, and review individualized resident care plans with the team.
Coordinate admissions, transfers, and discharges per procedures.
Participate in QA activities and staff education.
Complete clinical documentation per policy.
Perform other duties as assigned by the Director of Nursing.
