LPN or RN Schedule – every Friday and Saturday 10 p.m.–7 a.m.
Homecare with Heart offers the most competitive pay scale in the area. We’re looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will be responsible for overseeing the quality and expert care of your assigned home‑care patients. Starting rate for LPN: $26 per hour, RN: $30 per hour depending on experience.

Responsibilities

• Administer medication in accordance with standards of practice.
• Encourage social habilitative skills and self‑care and independence.
• Perform client treatments in accordance with the Plan of Care.
• Assist the consumer in learning appropriate self‑care techniques.
• Prepare clinical and progress notes to document care provided.
• Monitor, document, and report any change in client condition to the Clinical Director/Clinical Supervisor.
• Promote activities to achieve a safe and healthy environment.
• Observe aseptic technique when performing client procedures/care.
• Collaborate with other members of the clinical team to develop and improve the Plan of Care.
• Assist the client in achieving established goals in the Plan of Care developed by the RN.
• Maintain productivity according to the standards of the agency.
• Maintain license, CEUs and mandatory employment requirements.
• Participate in client‑care meetings, in‑services, orientation and staff meetings.
• Provide service in accordance with the agency’s philosophy, standards, policies and scope of practice.
• Participate in Performance Improvement and adhere to HIPAA guidelines.
• Other duties as assigned.

Qualifications

• ValidState LPN/RN License.
• Valid driver’s license; automobile use required due to travel.
• Annual MUI (Incident) training as required by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities.
• Ability to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.
• Proficiency with a computer, laptop or tablet.
• Independent thinker who can recognize potential problems, gather information, and know when to contact a supervisor.
• Demonstrated commitment to excellent customer service and exceptional client care while adhering to agency and program rules.
